Incidents and Accidents
Ensure > I&A > Critical flag When reporting an I&A, it can be defined as 'Critical'. This is then always at the top of the list of I&A, in red, when viewing all I&A.
It can only be removed from the 'Critical' list by the Safety Admin. A Critical Incident
email is generated, (over and above the existing email alerts), and is sent to people defined on the Critical Incident list.
A note is added to the data entry section that defines a Critical Incident:
A critical incident is an event posing an immediate, severe threat to health, safety,
security, or significant operational disruption, requiring urgent response.
Incidents and Accidents > Managers Ensure > I&A > Reporting Injury Forced to Escalate when an Injured Person is selected as one of the Primary
Categories.
Ensure > I&A > View, Edit, Escalate, Close > Filters When viewing reported I&A, you can now filter all Incidents and Accidents by
category.
Ensure > I&A > View existing I&A > New filters When viewing an Escalated Incident / Accident, you can now use a filter to skip between the different pages of the I&A report.
Ensure > I&A > View, Edit, Escalate, Close > Email error flag When viewing reported I&A, you will now see a red triangle next to any incident where an email error exists in the log.
Incidents and Accidents › Safety Admins
Ensure > I&A > Exporting results > New CSV KPI data When exporting Incident / Accident results, you can now select 'CSV - KPI Data' as am option. This exports just the key KPI data for any Board reporting, without all the additional data.
Ensure > I&A > Safety Admin > Request for Escalation Safety Admin can now request for a logged Incident/Accident to be escalated. Triggering an Escalation request email to the location. Once escalation requested, location admin cannot close accident. Safety Admin can also resend Request for Escalation email. In the view all I&A list, I&A are flagged as being 'Escalation
Requested'.
Ensure › I&A › Critical incident types Clients can designate certain incident types as critical by default, regardless of the
priority assigned by the person reporting the incident.
Ensure > I&A > Critical incident email Added the Title and Description of I&A to the Critical Incident email to give more headline information to recipients.
Ensure > I&A > View, Edit > Insurer reference field Managers can now enter an Insurance reference number field on the 'Initial report'
page (if a case number is subsequently provided by the insurer). The number entered will appear on the csv download of all I&A. Ensure > I&A > Email to Insurer limited to one If an existing escalated I&A is re-submitted, the insurer (if defined) will not be notified with a second email, to avoid the scenario of duplicate cases being created for the same I&A.
Ensure › I&A › Resend email to Insurer Safety Admin can choose to trigger a second Escalated I&A email to the Insurer.
Corrective Actions
Ensure > Corrective Actions > Ticket logged Managers can now mark Corrective Actions as 'Ticket logged' and enter a ticket number. Corrective Actions that have this flag are excluded from the Ensure Score.
Ad hoc Checks & Events
Ensure > Ad hoc Check & Events > Email alert When a location completes an 'Ad Hoc event or Check', an email can be sent to a designated recipient. For instance, after a "Regulatory Visit Event," an email notification is sent to the Legal department.
